Kratom withdrawal — what to expect.

Kratom withdrawal typically starts 12–24 hours after the last dose, peaks on days 2–4, and resolves in 7–10 days for leaf-kratom users. Symptoms: muscle aches, restless legs, insomnia, anxiety, runny nose, diarrhea. Uncomfortable but not medically life-threatening. 7-OH products produce a much more severe withdrawal and deserve medical supervision.
Phase-by-phase symptoms
0–12 hours
Baseline. Irritability creeping in. Cravings.
12–48 hours
Restless legs, yawning, runny nose, poor sleep, mild anxiety, anhedonia. Physical discomfort building.
Days 2–4 — peak
Muscle aches (especially lower back and legs), diarrhea, stomach cramps, sweating, insomnia, piloerection ("goosebumps"), panic-like episodes. This is when most relapses happen.
Days 5–7
Physical symptoms fading. Sleep improving but still fragmented. Mood still low.
Days 7–14
Physical symptoms mostly resolved. Motivation and appetite return. Psychological cravings continue but milder.
Post-acute (days 14–30)
Subtle mood dysregulation, sleep variations, and dopamine recovery. By day 30, baseline restored for most.
Management toolkit
- Hydration + electrolytes. GI losses cause real dehydration.
- Loperamide (Imodium). OTC dose for diarrhea.
- Ibuprofen. For muscle aches.
- Clonidine (prescription). Reduces autonomic symptoms (sweating, restlessness). Requires blood pressure monitoring — get a prescription.
- Gabapentin (prescription). Off-label for sleep + anxiety during withdrawal. Physician-guided.
- Hot baths, exercise, sunlight. Endorphin and mood restoration.

Leaf kratom withdrawal typically peaks on days 2–4 and resolves in 7–10 days for most users. 7-OH product withdrawal is longer and more severe — 14–28 days. Post-acute symptoms (low mood, sleep disruption) can linger up to 30 days.
Is kratom withdrawal dangerous?
For leaf kratom alone: uncomfortable but not medically dangerous. Does not cause seizures like alcohol/benzo withdrawal. For 7-OH products, taper under medical supervision. Seek help if you have a history of seizures, cardiovascular disease, or are polysubstance-dependent.
How long before withdrawal starts?

Will Suboxone help?
It can, but it's a major step. Suboxone creates its own dependence and requires medical supervision to start (precipitated withdrawal risk). Most leaf kratom users do not need it. For heavy 7-OH users, it is often the right choice — discuss with an addiction medicine physician.
Can I do this alone?
